Form 6 is issued by the Election Commission of India for citizens who want to apply for a new Voter ID card. If you are 18 years old or will turn 18 in the next few months, you can use this form to register as a voter. You can apply for your Voter ID by following the steps given below.
After SIR (Draft) Roll published, those voters whose name is removed or missing can apply for new voter card by filing Voter 6 Form.
Voter Form 6 - Apply Online for New Voter ID Card
STEP 1: Go to the Voters’ Services Portal using the link voters.eci.gov.in
STEP 2: At the homepage click on "Form 6" under New Voter Registration

STEP 3: After clicking on that link you have to click on "Sign Up" button

STEP 4: Now enter the mobile number and captcha code, then click on continue button
STEP 5: Then enter your First name and OTP received on your mobile to verify your profile and your user ID successfully created
STEP 6: Once your ID created, you can login with your mobile number, password and OTP on the portal homepage.
STEP 7: Select Fill Form 6 from the New Registration for General Electors section.
STEP 8: Fill your personal details, family details, address, DOB, Aadhaar and contact info. Upload supporting documents and choose Preview and Submit.

STEP 9: Then you have to fill the declaration form and check everything once again and enter the captcha code to Submit your Form 6 request.

STEP 2 Fill the form with personal details, address and date of birth details.
STEP 3 Submit the filled form along with documents and one passport-size photo.
Documents Required to apply for New Voter Card
- Proof of identity such as Aadhaar card, PAN card or any government ID which shows your correct name
- Proof of address like electricity bill, water bill, rent agreement or any document that proves where you live
- Proof of date of birth such as birth certificate, school certificate or Aadhaar showing DOB
- One recent passport size photograph for the form
Check Voter Registration Application Status
After submitting Form 6, people can check the application status online to know if their name is added to the voter list.
STEP 1 Open voters.eci.gov.in and login using your mobile number, password and OTP.
STEP 2 Select Track Application Status from the dashboard.

STEP 3 Enter your reference ID number and click on Submit button.
STEP 4 Your voter registration status will appear on the screen.
