Filling the form online takes just a few minutes if you have your mobile number and EPIC number ready. Follow these steps carefully.
STEP 1 - Go to the official ECI voters portal at voters.eci.gov.in and sign up using your mobile number. New users must create an account first.
STEP 2 - Enter the OTP sent to your registered mobile number to verify your login.
STEP 3 - Log in to your account and enter your EPIC number (Voter ID number) to pull up your existing details.
STEP 4 - Open the SIR Enumeration Form and fill in all details correctly, including your name, address, date of birth, and family information.
STEP 5 - Upload the required supporting documents in a clear and readable format so they are not rejected.
STEP 6 - Complete the final verification using OTP or e-sign as prompted on the screen.

Required documents depend on your year of birth and place of birth. The rules are different for each group, so check which one applies to you before filling the form. Any document you provide must establish your date of birth and place of birth.
If born in India before 01.07.1987: Provide any document (for self) from the official list that establishes your date of birth and place of birth.
If born in India between 01.07.1987 and 02.12.2004: Provide any document (for self) and any document (for father or mother) from the official list that establishes the date of birth and place of birth.
If born in India after 02.12.2004: Provide any document (for self), any document (for father), and any document (for mother) from the official list. If any parent is not Indian, attach a copy of his or her valid passport and visa as it stood at the time of your birth.
If born outside India: Attach proof of Birth Registration issued by the Indian Mission abroad.
If you got Indian citizenship by Registration or Naturalisation: Attach your Certificate of Registration of Citizenship.
If your name is missing from the draft roll, you may also need to submit Form 6 along with these documents and a recent passport-size photograph. After submission, EROs verify unmatched entries using documents, address proof, family details, or a hearing before a name is added to the final roll.
