Undertaking / Certificate Text of the BLO Undertaking Form
I, the undersigned, certify that I have personally verified the electoral records and relevant documents of the voter mentioned above.
After verification, it appears that in the previous voter list (last SIR), the name of the above-mentioned voter/voter’s guardian was written differently due to spelling variations, or the addition/omission of a middle name or surname. After proper verification, I hereby confirm that the voter/guardian mentioned in the previous voter list and the current voter list are the same person.
This undertaking is issued for the purpose of ensuring correct mapping in the voter list.
